BACKGROUND:

Theaseptictechniquesofanesthesiologistsinthepreparationandadministrationofinjectedmedicationshavenotbeenextensivelyinvestigated,butemergingdatademonstratethatinadvertentlapsesinaseptictechniquemaybeanimportantcontributortosurgicalsiteandotherpostoperativeinfections.

METHODS:

Aprospective,open,microbiologicalauditofcasesinwhichanesthesiologistswereaskedtoinjectallbolusdrugs,exceptpropofolandantibiotics,througha0.2-μmfilterwasperformed.Theauthorsculturedmicroorganisms,ifpresent,fromthe0.2-μmfilterunitandfromtheresidualcontentsofthesyringesusedfordrawinguporadministeringdrugs.Participatinganesthesiologistsratedeaseofuseofthefiltersaftereachcase.

RESULTS:

Twenty-threeanesthesiologistseachanesthetizedupto25adultpatients.Theauthorsisolatedmicroorganismsfromfilterunitsin19(6.3%)ofcases(3caseswereexcluded),includingStaphylococcuscapitis,Staphylococcuswarneri,Staphylococcusepidermidis,Staphylococcushaemolyticus,Micrococcusluteus/lylae,Corynebacterium,andBacillusspecies.Theauthorscollectedusedsyringesattheendofeachcaseandgrewmicroorganismsfromresidualdrugin55ofthese2,(2.4%)syringesincludingalltheaforementionedmicroorganismsandalsoKocuriakristinae,Staphylococcusaureus,andStaphylococcushominus.Participantsaverageratingofeaseofuseofthefilterunitswas3.5outof10(0beingveryeasyand10beingverydifficult).

CONCLUSIONS:

Microorganismswiththepotentialtocauseinfectionarebeinginjected(presumablyinadvertently)intosomepatientsduringtheadministrationofintravenousdrugsbybolusduringanesthesia.Therelevanceofthisfindingtopostoperativeinfectionswarrantsfurtherinvestigation
